 * Account received by date and volume
 * Amounts collected, by account
 * Percentage of total received versus
    collected, by account
 * Percentage of $$ collected versus
    originally placed
 * Complete Monthly Report which includes
         **Members Name
         **Your Contract Number
         **Original amount placed
         **Amount Collected
         **Date payment was received at Agency
         **Payment Type (credit card, cash, check)
         **Commission Amount
         **Dollar amount to be applied to Account
